What body surface area is used for

BSA estimates the total skin surface of a person from just height and weight, and medicine leans on it everywhere scaling matters: chemotherapy dosing, cardiac output indexed to body size, glomerular filtration, burn assessment, and ventilator settings. An average adult man lands near 1.9 m² and an average adult woman near 1.6 m².

Choosing between the formulas

Mosteller (√[height × weight / 3600]) is the modern favorite because it is easy to compute by hand and agrees closely with the older formulas. Du Bois from 1916 is the classic, still hardcoded in many dosing protocols. Haycock was derived across children and adults, and Gehan & George from oncology data also handles pediatric patients well. For a typical adult they differ by well under 3%, so any of them is defensible unless a protocol specifies one.

Does BSA work for children and infants?

Yes, and it matters more: pediatric drug doses are usually scaled per square meter. The Haycock and Gehan & George formulas were fitted across a wide age range and track children especially well.

Many drugs — chemotherapy in particular — have a therapeutic window tied to metabolic mass rather than raw weight. Dosing per m² of body surface historically tracked clearance better than weight alone, which is why oncology protocols specify a target BSA.
