Index vs. course handicap
Your handicap index measures potential ability on a standard fictional course with slope 113. Real courses are harder or easier, so the course handicap rescales the index by the tee's slope rating: index 12.5 from tees sloping 125 plays as about 14. A higher slope means the course plays tougher for higher-handicap players, so you receive more strokes.
What the target score means
Adding the course handicap to the course rating estimates the score of a golfer playing exactly to their ability from those tees. Shooting at or under that number in a counting round tends to lower your index; the handicap system's best-8-of-20 averaging means one great or terrible round moves it only gently.
